These surveys are designed to assess immunization practices in Long Term and and Intermediate Care facilities in California, and should be completed by the person in charge of infection control in your facility. You can participate in any open survey whether or not you participated in the previous surveys.

Your responses to this survey will be kept strictly confidential, and your facility's responses will not be analyzed individually.

Only one survey response from each facility is required. We very much appreciate your participation.
