The tools in this kit can support efforts to improve influenza vaccination rates among health care workers. The template materials can be easily adapted for use in a comprehensive health care worker vaccination effort at any facility.

(Spanish and English)	California Department of Health Services	This bilingual flyer is a friendly reminder to health care personnel about the importance of annual influenza vaccinations. It can be delivered to staff mailboxes or posted in strategic locations throughout facilities, such as staff locker rooms and restrooms, nursing stations, staff bulletin boards, storage rooms, and employee health offices. There is space to add information about locations and dates for flu vaccination at your facility.
Template: Letter from Management (English and Spanish)	California Adult Immunization Coalition	Direct support from management is key to increasing health care worker participation in immunization efforts. Letters from chief officers are an easy way to demonstrate support. This bilingual template can be adapted to your organization and distributed to staff and volunteers.

The template can be adapted to any influenza effort and used by front-line managers to encourage staff participation in the immunization effort.
Template: Declination of Flu Vaccine (English and Spanish)	California Adult Immunization Coalition	Use of a declination form is encouraged as a way to identify the reasons why health care workers decline vaccination. Similar forms have been used in many institutions to encourage and record Hepatitis B vaccinations. The template can be adapted for any site and used with staff and volunteers.

Publication: Influenza Immunization Among Health Care Workers - Call to Action	National Foundation for Infectious Diseases	This 4-page pamphlet illustrates the proportion of health care workers not receiving annual influenza vaccinations and the serious medical consequences that can arise from unintended transmission of the virus from worker to patient. The pamphlet can be used to educate decision-makers who institutionalize minimum standard of care infection control policies within hospitals, clinics, nursing homes and other health facilities.
